We often get asked on first aid courses what to do if they find someone in a completely different position.
It’s a very fair question as training scenarios often start with someone neatly lying on their back, ready for checks and treatment. Real life can be much messier than that.
So what happens if you find someone unconscious and lying on their front?
Start With Your Checks
As always, begin with your normal checks:
- Danger
- Response
- Airway
- Breathing
- Call for help
Those principles don’t change.
Do You Need to Roll Them Over?
Not always. If someone is already on their front or side, trying to roll them fully onto their back may be awkward, unnecessary, or even make things more difficult.
Sometimes, small adjustments are all that’s needed.
Helping Them Breathe More Easily
In our latest Trainer’s Corner video, Emilia explains how moving the casualty’s:
- hand under the head
- knee out to the side
- adjusting the head position
can help lift pressure off the chest and allow the ribs to move more freely.
That can make breathing more effective while still helping keep the airway clear.
Why Position Matters
If the chest is pressed flat against the floor for too long, breathing may become more difficult.
That’s why simple repositioning can make a real difference.
The Key Reminder
There’s no point placing someone into a safe airway position if you haven’t checked they are breathing.
Breathing comes first. Always make sure you’ve checked the airway and breathing before leaving to get help.
Real-Life First Aid Is Rarely Perfect
First aid isn’t about perfect textbook positions.
It’s about:

🔸Doing what’s safest in the moment
